The Irish Time on Friday carried a report on an inquest into the death of a woman in Drogheda. An ambulance crew was dispatched to the wrong address and spent valuable time trying to get into the wrong house while a woman lay dying of a heart attack. Root cause: The woman's address and the address the ambulance went to were similar. The woman's address was not listed the SatNav system used by the emergency services. They went to the other address because it was listed. The IQ issues here - information completeness and information accuracy. While the delay was held by the Coroner not to have contributed to this woman's death, it is likely that similar innaccuracies and issues exist in the emergency services data. The emergency crew spent valuable time trying to get into the wrong house - was there a process step missed that could have alerted them to the address error more quickly? Would a proper post code system for Ireland have helped avoid this problem?
